NSSLHA wants to help you in your application process to graduate school at Sacramento State University. Below, please find the forms needed to apply to the master's program for speech-language pathology and audiology.

Make sure to attend our meeting on September 23, 2009 at 7am in Shasta 240 to hear Dr. Boles, the Graduate Advisor, speak about the application process for prospective students.

Attached below is Dr. Roseberry's form to request a letter of recommendation from her. She requires 3 weeks (21 days) of prior notice for writing you a letter of recommendation. She only accepts requests during the regular school year -- not on vacations.
